Privacy collection notice

GV Complete Care collects the personal information in this form so we can respond to your enquiry, complaint or referral, and where needed assess, arrange and provide supports. If you do not provide the requested information, we may not be able to assist you properly. We may share your information with relevant staff, contractors, the NDIA or plan manager, health practitioners, or others where required or authorised by law. You may make a complaint anonymously if you prefer.

How We Handle Complaints

All complaints are treated confidentially and reviewed by our management team. We aim to acknowledge your complaint within 2 business days and provide a resolution within 10 business days. If you are not satisfied with the outcome, you may escalate your concern to the NDIS Quality and Safeguards Commission.
